• Full names, addresses and occupations of the person(s) you want to be the Trustee(s) of the trust. This is the person who will administer the trust.
• Full names, addresses and occupations of the person(s) you want to be the Beneficiaries of the trust. The person who will receive income or assets from the trust.
• Full names, addresses and occupations of the person(s) who the assets go to in the event all the Beneficiaries pass away.
• The name you would lilke to call the trust.
